Hospice care is a special kind of care that focuses on the quality of life for those experiencing an advanced, life-limiting illness. Hospice provides compassionate care for people in the last phases of an incurable disease so that they may live as fully and comfortably as possible, typically in the comfort of their own homes.
Some of the hospice benefits covered include: comforting care provided by an interdisciplinary team; pain and symptom management; psychosocial and spiritual support; medications; equipment and supplies; patient and family education; comforting treaments; therapies and bereavement services.
If you feel you or your loved one would benefit from hospice care, please reach out to your primary care physician to initiate that discussion. If your physician agrees and sees the need for hospice care, they can directly send a referral to Axiom Hospice. If you do not have a physician, you can call our office and our team will be glad to have a visiting physician come to your home and review your medical history. After their evaluation, if they feel hospice is appropriate, they can then send a referral to Axiom Hospice to initiate care.
We will work with you and your physician to determine eligibility. The Medicare and Medicaid Hospice Benefits cover services related to the terminal prognosis*. We will assist in checking your benefit coverage to make sure you understand any potential out-of-pocket costs. Typically all hospice services are covered by Medicare and Medicaid, as well as many private insurances, with no out-of-pocket costs for eligible beneficiaries. Hospice also covers medication, medical supplies an medical equipment related to the hospice diagnosis*. Managed Medicaid programs and HMOs may require co-pays and deductibles for hospice care.
