Our transition liaisons will visit you bedside at the hospital or at the skilled nursing facility to explain our services, answer any questions you might have and gather critical information to ensure a smooth transition upon your discharge.
According to Medicare, 99% of our cases are opened within the Medicare guidelines (24 hours after discharge from hospital/skilled nursing facility or 48 hours after a referral is sent from a physician office). Getting out to see our patients quickly helps us prevent unnecessary hospitalizations and help our patients gain their independence quicker
Our teams communicate frequently to ensure everyone is on the same page of care for our mutual patients. If one team member notices any abnormality, they make sure others are also aware and can take proactive steps, in order to prevent any unnecessary hospitalizations or patient decline
Our team members involve your physicians in the care discussion in an effort to coordinate care. Whether it is a primary care physician, a surgeon or a specialist (cardiologist, nephrologist etc.), our team will communicate key findings to the physician and make sure they are involved with any changes that are made.
Axiom offers specialized programs that are catered to your individual needs. These programs have been proven to control symptoms and help reduce complications and unnecessary hospitalizations.
An Axiom staff member is on call 24/7 and can triage your concerns and have a RN case manager follow up with you after hours. This has been helpful in stabilizing a patient till a staff member can come out to see the patient and prevent unnecessary hospitalization.

Home Health Care is customized health care and education provided to patients in the comfort of their own home. It is typically provided on a temporary and intermittent basis following a hospitalization, rehabilitation stay, or a change in health condition. A home care episode may include a combination of nurses, physical therapists, occupational therapists, speech pathologists, medical social workers, and home health aides.
Patients must be considered homebound. This means getting out of the house exerts a considerable taxing effort and may require the assistance of another person or a medical device such as a walker, cane, or oxygen. (An individual receiving home health care may leave the home for short infrequent trips such as church services, doctor visits, trips to the grocery store, hair appointments, or special family events, but generally the individual is confined to the home by their medical condition.)
Skilled nursing care or rehabilitation therapy is medically necessary, reasonable, and ordered by a physician who has current knowledge of your medical condition.
Skilled home care services are meant to be provided on an intermittent basis rather than on a daily basis.
Eligible Medicare or Medicare Advantage recipients can expect Home Health Care will be covered 100%. There are no copays or deductibles on the Medicare benefit for home health care. Many other insurances, such as Tricare and auto accident insurances, are also covered in full when appropriate criteria are met. Insurance benefits will be verified on an individual basis by the Axiom Home Care intake department prior to the start of the home health care episode. Some Medicare advantage plans and other commercial insurances will require prior authorization by the agency. Some insurance plans have limited provider networks. Patients will be notified prior to the last visit that their care will be coming to an end.
Heart disease, CHF, Diabetes, Wound Care and vascular conditions, COPD, Pneumonia, Hypertension, Arthritis, Strokes, Parkinson ’s disease, Dementia, catheter care, post-surgical care, and for treatment of infections requiring IV antibiotics.
A nurse or rehabilitation therapist will call to schedule a mutually convenient time for the assessment visit. The first visit may last from 1-2 hours and will serve as a comprehensive assessment. This visit will be different from other visits. A care plan will be established with an estimate of the number of visits of each type that will be anticipated to achieve your goals. Following the first visit, your physician will approve the plan of care paperwork and make adjustments as needed.
A home health care episode may run for up to 60 days with intermittent scheduled appointments. The frequency of nursing and rehabilitation visits is customized based on physician orders and individual patient goals and needs. Visits may include teaching instructions for caregivers as well as patients.
Visits are typically less than one hour to allow time for appropriate documentation following the face time in the home.
Medicare does not reimburse for custodial, companion services, or hourly care.
There is no limit to the number of 60 day home health care episodes a qualifying candidate can receive in a year or a lifetime under current Medicare rules (as of January 2017). If a physician agrees that home care is reasonable and necessary, it can be a good way to help avoid more expensive and traumatic hospitalisations and E.R. visits.
Yes. Regardless of who recommends you for home care services, you have the right to choose who you prefer as your home care provider. Even your hospital discharge planner cannot force you to accept a particular agency. Likewise, your physician can make recommendations, however, you have the right to make the final decision
Yes. If a patient has dementia, our staff works closely with the patient and the caregiver/family member to ensure understanding of the patients abilities and ways to manage them
No, if you are referred to our agency by a hospital/facility physician, we will reach out to your primary care physician and/or specialist to coordinate care. If your referral was initiated by your primary care physician, we will continue working with them. At no point of time are you setup with a different doctor.
No. Studies have found that patients have a higher recovery rate in the comfort of the own home. The level of care from Axiom is equal to what is provided in an institution. If our staff feels that institutional care is required, we will work with the patient and their primary care physician to arrange for the appropriate support.
Yes. While most home care agencies only provide nursing and therapy care, Axiom Home Care also provides services such as Home Health Aides, Medical Social Workers and Speech Therapy. Our staff can aide in arranging for community resources such as chore workers, Meals on Wheels and other government provided assistance programs. Our aides also will help our patients shower/bathe and provide personal care based on insurance guide lines
Yes. Our agency rigorously recruits top of the line clinicians. Axiom staff are professionally licensed and must possess experience within different clinical areas. Axiom Home Care is proudly accredited by the Better Business Bureau
Yes. A patient is considered eligible for home care if they have difficulty ambulating independently. However, you can leave your home in instance of doctor appointments, church, grocery shopping and for personal care.
Yes. Home care is an excellent opportunity for a caregiver to learn how to manage a patient’s needs. The caregiver is provided with the tools to ensure they are able to offer the most appropriate care.
